We have not received FULL payment from your insurance for your Covid medical visit to AFC Urgent Care, Newark, CA.
AVOID collections or Court action.

Throughout the COVID pandemic we were at the forefront serving all clients / patients every day 7 days a week, 365 days a year. We are a private urgent care that offered Covid services with NO OUT-OF-POCKET COST, which under the FEDERAL CARES ACT, ALL COVID BILLS ARE TO BE PAID BY INSURANCE COMPANIES OR FEDERAL GOVERNMENT or STATE OF CALIFORNIA.
In accordance with federal law, there is no surprise billing issue, as we posted at the office lobby and web page, the fact that we are out of network with all insurance companies and that patients are responsible for the whole bill. We tried to reach an agreement with insurance companies on the covid bill, so patients are not charged anything out of pocket. The Federal Government published on the HRSA web site THAT THEY RAN OUT OF COVID FUNDS and refused thousands of OUR BILLS FOR THE UNINSURED that we served. SO, OUR OFFICE HAS BEEN HIT WITH A DOUBLE WHAMMY, REFUSAL OF THE FEDERAL GOVERNMENT TO PAY ALL THE UNINSURED BILLS AND THE REFUSAL BY INSURANCE COMPANIES TO SETTLE THE COVID BILLS.
We, as a courtesy, billed the insurance companies for their insured PATIENT. Under the Federal Cares Act all insurance companies were to pay the entire Covid bill for their insured. This was published on the Federal CARES ACT site and on private website Saloojasinc.com.
NO SURPRISE BILL. NO OUT-OF-POCKET COST PROMISE BY CARES ACT.
Insurance Companies shifted the burden to patient in violation of CARES act.
Unfortunately, the federal courts ruled that the Cares Act is not enforceable by provider of the Covid services, the DOCTOR and so insurance companies have simply rejected payments of our bills. In doing so they have shifted the responsibility of payment to the insured, YOU, the patient, which was not the intent of Congress when it passed the CARES ACT. We cannot sue the insurance companies directly for enforcement of the CARES ACT, only you the patient can sue Insurance company for breach of contract, when the insurance company does not pay the bill. We have been trying for 2 years to resolve the situation with the insurance companies but until we reach an agreement, all patients remain fully responsible for their whole bill which should have been paid by their insurance company but was not.
DOLLAR AMOUNT DUE
The exact amount that you owe the doctor's office will depend on your health PLAN and the insurance company negotiations with you, the PATIENT, or the DOCTORS office. So far insurance companies are still trying to deny the bill in full or partially. Hence the exact amount can be full charge as billed to Insurance or as per agreement between the insurance based on your PLAN and you the patient, that is offered in settlement. As explained, this amount can vary based on your specific health plan as BRONZE, GOLD, PLATINUM, PPO, HMO, Medi-cal.
EOB or EOP sent to you by Insurance company.
Details of processing by the insurance company were sent to you as Explanation of Benefits (EOB) or Explanation of Payment (EOP). You received the EOB or EOP in the mail from the insurance company. In some cases, insurance wrote you a check, that you may have cashed. Given that you are responsible for the bill to the doctor’s office, you are requested to share a copy of your EOB or EOP with us through the email. If you did not receive an EOB or EOP, you can ask insurance for such document or look it up on the insurance portal. Depending on your specific plan,
you may owe up to $1100 or more per visit.
RESOLUTION
TO TRY TO RESOLVE this situation, we offer to the patients being manipulated by their insurance company as follows:
1. We request your cooperation to talk to your insurance company in the next 180 days, subject to, you can pay one good faith deposit $100 Today and then negotiate with your insurance company to pay the bill by June 30, 2024 as set forth under the CARES ACT. If the insurance company pays or reaches an agreement with us, then there is no cost to you which was the intended result by Congress under the CARES Act. In that case your $100 would be refunded to you after insurance settles the bill
OR
2. You can pay the full amount as explained TO YOU BY YOUR INSURANCE by EXPLANATION OF BENEFITS (EOB) PAPER RECEIVED BY YOU FROM THE INSURANCE COMPANY and then sue your insurance company in small claims for breach of contract and damages caused by its failure to pay the bill as required under the CARES ACT.
FURTHER ACTION
If you do nothing, we will be forced to file small claims action against you for collection for the full billed amount, effective Jan 1, 2024, along with costs of said suit. You can still sue the insurance company for reimbursement.
We urge you to settle with your insurance so last resorts as lawsuits can be avoided for all parties.
ACTIONS WITH GOVERNMENT AND INSURANCE COMPANIES
You are welcome to share this web page with the insurance company that is denying payments and share it with federal senators, federal congressman for failure of the CARES act. Also share with the California Governor’s office and other government officials as the health insurance fails to protect the people.

NO OUT OF POCKET COST ENDED for covid testing or vaccines.
All Patients are personally liable for all Bills regular or covid bills from our offices at SALOOJASINC, AED Urgent Care (Previously known as AFC urgent Care). The No out of pocket cost for covid is valid, if your insurance pays the covid bill as required by the Federal cares Act.
If you get a bill for Covid, then you are required to call the insurance and have the insurance pay the bill to the Medical Office or You the patient need to pay the bill. If the bill is not paid within 30 days of receipt of bill, we reserve the right to sue the patient and insurance for the bill.
The details below clarify the position further.
If a health plan enrollee receives a bill related to the coverage of a COVID-19 test, the enrollee should submit the bill to their health plan. If the health plan does not reimburse the enrollee in a timely manner, the enrollee should file a grievance with their health plan and include a copy of the bill.
